Case Manager​

Part Time- Entry Level Position

Job Summary:
We are seeking a dedicated and compassionate Case Manager to join our team. As a Case Manager, you will be responsible for providing comprehensive support and guidance to foster families. Your role will involve assessing needs, developing service plans, coordinating services, and advocating for foster children. Duties:

• Conduct comprehensive assessments of clients' needs, including physical, emotional, and social factors

• Develop individualized service plans in collaboration with clients and their support systems

• Coordinate and monitor the delivery of services to ensure clients' needs are met effectively

• Provide crisis intervention and support during emergencies or challenging situations

• Collaborate with community resources to connect clients with appropriate services such as housing assistance, therapeutic programs, childcare options, behavioral health services, etc.

• Supervise and monitor client progress towards goals outlined in their service plans

• Provide mentoring and support to clients through regular check-ins and motivational interviewing techniques

• Advocate for clients' rights and access to necessary resources

• Maintain accurate and up-to-date documentation of client interactions and progress

Qualifications:

• Bachelor's degree

• Previous experience in case management or related field preferred

• Strong interpersonal skills with the ability to build rapport with diverse populations

• Excellent communication skills, both written and verbal

• Ability to work independently as well as part of a team

• Proficient in using computer systems for documentation purposes
